Job Description: Disability Support Worker (Medium–High Support Needs)

Purpose of the Role

Provide personalised, safe, and empowering support to people with disability who require medium to high levels of assistance. The role focuses on enhancing independence, dignity, wellbeing, and community participation while delivering care aligned with individual support plans.

Candidates must hold valid work authorisation to work in Australia.

Key Responsibilities

1. Personal Care & Daily Living Support

Assist with activities of daily living (ADLs): showering, dressing, grooming, toileting, continence care.

Support with mobility, transfers, and use of assistive equipment (hoists, wheelchairs, walkers).

Prepare meals, support with eating and hydration, and follow dietary requirements.

Maintain a clean, safe, and comfortable living environment.

Minimum 6 months experience in disability support is required.

2. High‑Level Care Tasks (as required and within competency)

Support with medication administration following organisational policies.

Monitor health changes and escalate concerns appropriately.

Assist with complex behaviours using positive behaviour support strategies.

Provide support for participants with chronic health conditions, epilepsy management plans, diabetes care or other specialised needs (within training scope).

3. Community Access & Social Participation

Support participants to engage in community activities, appointments, education, and employment.

Encourage independence and skill development (e.g., budgeting, cooking, communication skills).

Promote social inclusion and meaningful engagements

4. Documentation & Reporting

Accurately record progress notes, incident reports, and changes in participant needs.

Follow NDIS and organisational compliance requirements.

Maintain confidentiality and uphold participant rights.

5. Safeguarding & Risk Management

Identify and mitigate risks to participant safety and wellbeing.

Follow behaviour support plans, risk assessments, and emergency procedures.

Uphold duty of care and mandatory reporting obligations.

Current police check, NDIS worker screening, and working with vulnerable people check are required.

6. Communication & Collaboration

Work collaboratively with families, allied health professionals, coordinators, and other support staff.

Communicate respectfully and adaptively with participants, including those with communication challenges.

Participate in team meetings, training, and supervision.
