You will deliver programs, provide guidance, connect clients with essential services, maintain records, liaise with agencies, and mentor junior staff.</p><br><h3>Benefits</h3><br><h3>Task & responsibilities</h3><ul><li>Work directly with clients to assess needs, strengths, and goals, and design tailored support or care plans.</li><li>Deliver and coordinate programs, workshops, and one-on-one sessions to build life skills, independence, and social participation.</li><li>Provide ongoing mentoring and coaching to clients, families, and carers, offering guidance through life challenges and transitions.</li><li>Refer clients to appropriate external services such as health, housing, counselling, employment, and disability support.</li><li>Maintain accurate client records, track progress, and prepare reports for internal use, stakeholders, and funding bodies.</li><li>Liaise with government agencies, community groups, and service providers to advocate for clients and coordinate support.</li><li>Promote awareness of community services through outreach, events, and information campaigns.</li><li>Oversee and support junior staff or volunteers to ensure consistent and responsive service delivery.</li></ul><br><h3>Qualification & experience</h3><ul><li>Diploma of Community Services.</li><li>Minimum 1 year of full-time work experience as a Community Worker in a similar environment.</li><li>Strong communication, organisational, and interpersonal skills.</li><li>Ability to work with diverse cultural backgrounds.</li><li>Ability to work efficiently under pressure.</li><li>Excellent written and verbal communication skills.</li><li>Strong decision-making skills.</li></ul>
