Your duties will involve helping clients with personal care, mobility, and meal preparation, while encouraging independence and respecting their dignity and preferences.Our care team works closely with each client to develop and follow personalised care plans that align with their physical, emotional, and social needs. This role requires compassion, adaptability, and a strong understanding of client safety and support practices. We are looking for individuals who are driven to provide high-quality care and contribute positively to the overall wellbeing of our clients.&nbsp;DUTIES &amp; RESPONSIBILITIES•&nbsp;&nbsp;&nbsp;&nbsp;Participating in planning the care of individuals•&nbsp;&nbsp;&nbsp;&nbsp;Assisting patients with their mobility and communication needs•&nbsp;&nbsp;&nbsp;&nbsp;Assisting patients with their personal care needs such as showering, dressing and eating•&nbsp;&nbsp;&nbsp;&nbsp;Following therapy plans such as interventions to assist those with dementia and behavioural problems•&nbsp;&nbsp;&nbsp;&nbsp;Providing direct support and assistance to therapists•&nbsp;&nbsp;&nbsp;&nbsp;Assisting with rehabilitation exercises, basic treatment and delivering medications•&nbsp;&nbsp;&nbsp;&nbsp;Observing and reporting changes in patients' condition, and reporting complaints about care•&nbsp;&nbsp;&nbsp;&nbsp;Collaborating with other healthcare professionals, family members, and support workers to ensure a holistic approach to care•&nbsp;&nbsp;&nbsp;&nbsp;Check resident’s daily menu requirements•&nbsp;&nbsp;&nbsp;&nbsp;Assist with medication reminders as per care plans•&nbsp;&nbsp;&nbsp;&nbsp;Support residents with daily activities and encourage social engagement•&nbsp;&nbsp;&nbsp;&nbsp;Maintain a clean, safe, and comfortable living environment for residents•&nbsp;&nbsp;&nbsp;&nbsp;Foster positive relationships with residents, families, and the care team&nbsp;QUALIFICATIONS &amp; REQUIREMENTS•&nbsp;&nbsp;&nbsp;&nbsp;AQF Certificate II or III, or at least one year of relevant experience (ANZSCO Skill Level 4)•&nbsp;&nbsp;&nbsp;&nbsp;Ability to work and communicate effectively within teams•&nbsp;&nbsp;&nbsp;&nbsp;Current full-time work rights in Australia•&nbsp;&nbsp;&nbsp;&nbsp;Ability to develop customised care plans and assist individuals with diverse needs•&nbsp;&nbsp;&nbsp;&nbsp;The ability to assist with everyday living tasks and provide personal care with competence•&nbsp;&nbsp;&nbsp;&nbsp;Knowledge of safety regulations and the capacity to adhere to them to maintain a secure and encouraging environment•&nbsp;&nbsp;&nbsp;&nbsp;Excellent time-management and organising skills•&nbsp;&nbsp;&nbsp;&nbsp;Assist clients in building and maintaining connections with friends, family, and the larger community•&nbsp;&nbsp;&nbsp;&nbsp;Relevant qualifications in Disability Support, Aged Care, or a related field.&nbsp;LOCATIONClient’s own home within the geographic areas of Melbourne, Shepparton, Geelong, Ballarat, Daylesford.&nbsp;&nbsp;WORKING HOURSFull-time position, 38 hours per week.&nbsp;BENEFITS•&nbsp;&nbsp;&nbsp;&nbsp;Competitive salary based on experience along with 12% superannuation.•&nbsp;&nbsp;&nbsp;&nbsp;Opportunities for professional growth and development.•&nbsp;&nbsp;&nbsp;&nbsp;Paid time off and holidays.•&nbsp;&nbsp;&nbsp;&nbsp;Flexible work schedule.&nbsp;
